In recent months, I’ve noticed more and more people asking questions like:
“Can ChatGPT help me manage anxiety?”
“Are there apps that act like therapists?”
“Is AI good for mental health support?”
Honestly, I understand the curiosity. Finding a therapist can be challenging, especially when waitlists are long. However, I think it’s time we have an honest conversation about what AI can do for mental health… and what it absolutely cannot replace.
Why People Are Turning to AI for Mental Health Support
It’s no secret that we’re living in an age of instant solutions. When someone is struggling emotionally, the idea of having a 24/7 chatbot who listens without judgment feels comforting. AI tools like ChatGPT are increasingly being used for:
- Emotional venting
- Journaling prompts
- Guided meditations
- Positive affirmations
Some people say it helps them feel seen, and in the absence of a therapist, even that little bit of validation can be a lifeline. I heard one lady say “Chat GPT has told me the nicest thing anyone has ever said to me”
I get it.
But as a mental health professional, I feel responsible for reminding people: AI is a tool, not a therapist.
What AI Is Actually Trained In (and What It’s Not)
Current large language models like ChatGPT are trained on vast amounts of text, including materials on Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT). That means it can sometimes mimic CBT-style prompts or offer DBT-inspired affirmations like:
- “Try to reframe that thought…”
- “Remember, emotions are not facts.”
But here’s the truth: AI doesn’t understand context.
It doesn’t know your history, nor does it recognize subtle cues, neither can it sense when you’re dissociating or shutting down. And it definitely doesn’t know when to refer you for more serious help.
AI cannot perform trauma-informed therapy.
There is no AI that can guide you through EMDR, somatic experiencing, or the complex healing work that trauma recovery demands. That’s sacred work—and it requires the presence of a trained human being who can hold space, not just generate a response.
The Risk of AI Validating Paranoia or Delusional Thinking
Another critical concern is what happens when individuals with paranoid thoughts or delusional beliefs turn to AI for “support.”
As a psychiatric mental health nurse practitioner, I’ve seen how difficult it can be for those experiencing psychosis to recognize their symptoms. Many have limited or no insight into their condition. If someone with paranoia types their fears into an AI chatbot, there is a real risk that the AI—unaware of the clinical picture—might respond in a way that unintentionally reinforces or validates those beliefs.
AI can’t assess for risk.
It can’t challenge irrational thinking with care.
It doesn’t know when someone is spiraling.
And without that clinical awareness, a well-meaning AI response can cause more harm than good.
This is not a small issue. It’s one more reason why licensed professionals are essential in mental health care—especially in complex or high-risk cases.
Evidence-Based Support for Human Therapists
There’s decades of research backing what we already know in our hearts:
Therapeutic alliance—the connection between a client and a therapist—is one of the strongest predictors of positive treatment outcomes.
According to a 2020 meta-analysis in Psychotherapy, the relationship itself accounts for as much as 30% of the effectiveness of therapy. AI simply cannot replicate that bond.
Trauma-informed care, especially, relies on:
- Co-regulation
- Trust and safety

When AI Is Not Enough: The Risk in Crisis Situations
There’s another reality we cannot ignore—AI is not equipped to respond appropriately to suicidal thoughts or mental health crises.
When someone expresses suicidal ideation in therapy, a licensed professional knows how to:
- Ask the right follow-up questions
- Assess for intent, plan, and means
- Determine the level of risk
- Implement a safety plan or initiate a referral for urgent care
AI, on the other hand, cannot assess risk. It can’t ask clarifying questions or notice nonverbal cues. It doesn’t know when someone is masking their distress—or when silence is a red flag.
Even when programmed with safety responses (like “please call 988”), AI doesn’t truly intervene. And in a crisis, hesitation or an automated answer can cost someone their life.
Mental health support isn’t just about having the “right words”—it’s about having the right training, presence, and awareness to act when someone is in danger.
This is why it’s so important to never rely on AI tools in place of human care—especially when dealing with depression, self-harm, or suicidal thoughts.
Where AI Can Be Helpful
That said, I don’t believe we need to reject AI altogether. It has its place—just not as a replacement for therapy.
You can use AI tools to:
- Generate journaling prompts
- Write affirmations tailored to your mood
- Practice mindfulness scripts
- Stay organized with your mental health goals
Think of it like this:
AI is your planner, your assistant, or even your motivator. But your therapist? That role belongs to a human being who can truly see you.
Final Thoughts: Human Healing Requires Human Connection
Mental health is a deeply personal and complex issue—not a one-size-fits-all conversation. It’s shaped by trauma, environment, biology, spirituality, relationships, and countless other factors. And while we’re grateful for the tools that technology offers, no AI can truly understand the layers of a human life.
Yes, AI can offer structure.
It can help us journal, reflect, or explore our thoughts.
